योग्यता : मेडिकल (एमबीबीएस/ बीडीएस/ बैचलर ऑफ आयुष आदि), पैरामेडिकल (बीफार्मा, बीएससी, नर्सिग, बीपीटी/ बीओटी आदि), साइंस और बिहेवियरल साइंस स्ट्रीम में ग्रेजुएशन।
आवेदन कैसे करें : प्रॉस्पेक्टस और आवेदन पत्र 200 रुपए का क्रॉस्ड इंडियन पोस्टल ऑर्डर या बैंक ड्राफ्ट दे कर हैल्थ प्रमोशन युनिट, डिस्टेंस लर्निग सेल, रूम नं. 417, एकेडमिक ब्लॉक, एनआईएचएफडब्ल्यू से प्राप्त किए जा सकते हैं। फॉर्म संस्थान की वेबसाइट से भी डाउनलोड किया जा सकता है।
